World Health Org (WHO) issues ethics guidance to protect rights of TB patients

GENEVA - New tuberculosis (TB) ethics guidance, launched by the World Health Organization (WHO), aims to help ensure that countries implementing the End TB Strategy adhere to sound ethical standards to protect the rights of all those affected.

TB, the world’s top infectious disease killer, claims 5 000 lives each day. The heaviest burden is carried by communities which already face socio-economic challenges: migrants, refugees, prisoners, ethnic minorities, miners and others working and living in risk-prone settings, and marginalized women, children and older people.

“TB strikes some of the world’s poorest people hardest,” said Dr Margaret Chan, WHO Director-General. “WHO is determined to overcome the stigma, discrimination, and other barriers that prevent so many of these people from obtaining the services they so badly need.”

Poverty, malnutrition, poor housing and sanitation, compounded by other risk factors such as HIV, tobacco, alcohol use and diabetes, can put people at heightened risk of TB and make it harder for them to access care. More than a third (4.3 million) of people with TB go undiagnosed or unreported, some receive no care at all and others access care of questionable quality.

The new WHO ethics guidance addresses contentious issues such as, the isolation of contagious patients, the rights of TB patients in prison, discriminatory policies against migrants affected by TB, among others. It emphasizes five key ethical obligations for governments, health workers, care providers, nongovernmental organizations, researchers and other stakeholders to:

provide patients with the social support they need to fulfil their responsibilities
refrain from isolating TB patients before exhausting all options to enable treatment adherence and only under very specific conditions
enable “key populations” to access same standard of care offered to other citizens
ensure all health workers operate in a safe environment
rapidly share evidence from research to inform national and global TB policy updates.
From guidance to action

Protecting human rights, ethics and equity are principles which underpin WHO’s End TB Strategy. But it is not easy to apply these principles on the ground. Patients, communities, health workers, policy makers and other stakeholders frequently face conflicts and ethical dilemmas. The current multidrug-resistant TB (MDR-TB) crisis and the health security threat it poses accentuate the situation even further.

“Only when evidence-based, effective interventions are informed by a sound ethical framework, and respect for human rights, will we be successful in reaching our ambitious goals of ending the TB epidemic and achieving universal health coverage. The SDG aspiration of leaving no one behind is centred on this,” said Dr Mario Raviglione, Director, WHO Global TB Programme.

“The guidance we have released today aims to identify the ethical predicaments faced in TB care delivery, and highlights key actions that can be taken to address them,” he added.

World TB Day is an opportunity to mobilize political and social commitment for further progress in efforts to end TB. This year, World TB Day signals new momentum at the highest levels with the announcement of the first ever Global Ministerial Conference on Ending TB, which will be held in Moscow in November 2017.

“The Global Ministerial Conference will highlight the need for an accelerated multisectoral response to TB in the context of the Sustainable Development Goals,” said Dr Ren Minghui, Assistant Director-General HIV/AIDS, Tuberculosis, Malaria and Neglected Tropical Diseases. “It will emphasize that global action against antimicrobial resistance must include optimized care, surveillance and research to address MDR-TB urgently”.

The Conference will inform the UN General Assembly high-level meeting on TB which will be held in 2018.

Thousands of jobs await the youth this summer in Philippines: Wellness in Economics

Thousands of jobs await the youth this summer: For a bigger number of youth who can avail of the program, there have been changes in the requirements.

“Ang tinatanggap lang ng SPES natin ay 18 to 25 so this year binaba natin to 15 dahil sa K to 12 program. “Ayon sa batas, dapat deserving and poor so yung annual family income hindi tataas sa P143,000,” said Deped undersecretary Jesus Mateo.

(In the previous years, members of the youth sector aged 18 to 25 are accepted in our SPES. This year we lowered the minimum age to 15 because of the K to 12 program. Based on the law, the applicants should be deserving and poor, so their annual family income should not exceed p143,000 pesos.)

Aside from students, the program is also open to the out-of-school youth who intend to continue their education.

The Special Program for the Employment of Students (SPES) is mandated under Republic Act number 9547. It aims to help the youth have money to spend for their education.

“Ang ibinibigay natin, ayon sa batas ay minimum wage which is Php 491 per day…so binibigaynatin ‘yan on a weekly basis.” USEC. Mateo added.

(The compensation we give them based on the minimum wage law is P491 pesos per day. We give it on a weekly basis.)

Those interested may apply in DOLE offices, public employment services offices or in other offices of the department which offer SPES. They just need to bring the following requirements:

SPES application form; birth certificate / baptismal certificate; latest income tax return of parents/ certificate of tax exemption/ certificate of indigency and form 138 or class cards (for students).

The deadline of application is on March 27, 2017"

Wellness in Tech: Pres. Duterte approves national broadband program

"Pres. Duterte emphasized the need for faster communications in the country," Piñol wrote. 

"Pres. Duterte earlier said he would like DICT 'to develop a national broadband plan to accelerate the deployment of fiber optics cables and wireless technologies to improve internet speed,'" he added. 

Salalima submitted three options to the Office of the President in October 2016 to implement a national broadband program. One of the options included installation of a national broadband network from scratch, which could cost the government P77 billion to P200 billion.

The DICT's national broadband plan was a response to Duterte's marching orders at his first State of the Nation Address (SONA) in July 2016, in which he pushed for free WiFi access to more areas in the countryside. 

A national broadband plan was also in the works during the time of former President and incumbent Pampanga Rep. Gloria Macapagal-Arroyo. 

The US$329 million National Broadband Network (NBN) deal with China's ZTE Corp. signed in April 2007 was cancelled by Arroyo in October 2007 following allegations of corruption and irregularities. — TJD, GMA News"

WHO News: Continues to work with Nigerian (Nigeria) Ministry of Health and partners to reach those in need with lifesaving health care

Years of violent conflict have deprived millions of people in north-eastern Nigeria, northern Cameroon, western Chad and south-east Niger of even the most basic health care services. Malnutrition is widespread and in the 3 worst-affected states of Nigeria nearly 6 million men, women, children and infants urgently need health assistance. WHO continues to work with the Nigerian Ministry of Health and partners to reach those in need with lifesaving health care.

The humanitarian crisis in north-eastern Nigeria has intensified after eight years of violent conflict. The crisis has evolved over the years leading to widespread displacement and devastation as well as acute food and nutrition insecurity and a desperate shortage of essential health care.

North-eastern Nigeria is at the heart of the larger Lake Chad Basin crisis (north-eastern Nigeria, northern Cameroon, western Chad and south-east Niger), which affects some 17 million people. WHO and partners are seeking to help 8.2 million people with health assistance across the region.

With less than half of its health facilities functional, Borno is the worst affected of the states in north-eastern Nigeria. Working with the Nigerian Government, WHO assessed the state’s 749 known health facilities and found that one-third were completely destroyed and another third were damaged. Of the health facilities that still function, 60% are supported by one or more of the 18 health sector partners.

24 mobile medical teams supported by WHO are deployed to the most remote and insecure parts of Borno State to provide urgently needed essential health care. The teams carry out more than 4000 consultations each week, including immunization, ante-natal care and testing and treatment of malaria. The teams respond to the movements of internally displaced persons and areas lacking health facilities to try and bring health services to those who need them most. 79 additional mobile teams are run by other health partners.

In Borno State, 160 health facilities use the WHO disease Early Warning Alert and Response System (EWARS). WHO has trained hundreds of health facility surveillance focal points to use EWARS since August 2016, enabling rapid detection and response to public health outbreaks for around 85% of the state’s 1.8 million internally displaced persons.

Millions of people in Borno State have had limited or no access to regular health services over a number of years, leading to very low vaccination rates. The ongoing movement of internally displaced people also makes it difficult to track those who have or have not been immunized. WHO supported a measles vaccination campaign in January 2017 which reached nearly 3 million children. Polio is still endemic in Nigeria, but WHO and its partners have vaccinated over 1.8 million children under five against the disease in a continued effort to eradicate it.

WHO has supported the training and deployment of almost 300 community workers. These volunteers provide essential malaria, diarrhoea and acute respiratory infection treatment to children under 5 in some of the most inaccessible areas of Borno state where formal health care structures are not functioning.

Constant conflict continues to challenge the abilities of WHO and health sector partners to access people who are most in need of basic health services. Insecurity is a major constraint, with recent attacks on humanitarian staff. Access to local government areas requires military escort on poor roads and communication with many of these areas is extremely limited.

Disease outbreaks will pose a greater threat as temperatures continue to rise over the coming weeks and months. The start of the rainy season in April will further heighten the risk of cholera and meningitis epidemics and cases of malaria, which has become endemic in the north-east of the country, are also expected to rise.

Malnutrition rates are well above emergency thresholds in many parts of north-eastern Nigeria. The need for food assistance is likely to increase even further next month when stores from last year’s poor harvest run out. According to the Nigeria-National Nutrition and Health Survey (NNHS-2015), 2.6 % of children aged 6-59 months in Borno State have Severe Acute Malnutrition (SAM). UNICEF warns of an estimated 450 000 child SAM cases in 3 states of the north-east in the next 12 months.

A constant threat of insecurity keeps people on the move. Eight years of conflict has forced more than 2.3 million people to flee their homes throughout the Lake Chad Region, sometimes multiple times. With a population in flux, it is difficult to ensure that people have regular access to the basic health services they need.
